This member of the Housing First Team and the Refuge Transition Team is responsible for conducting psychiatric assessments; assessing physical health needs; making appropriate referrals to community physicians; providing management and administration of medication in conjunction with the psychiatric nurse practitioner; providing a range of treatment, rehabilitation, and support services. The LPN provides assistance to the psychiatric nurse practitioner and coordinates health care with the client’s primary care provider to ensure continuity of care. The LPN is responsible for notifying primary care providers of services provided by the Housing First Team and the Refuge Transition Team.

EDUCATION and/or EXPERIENCE:
Licensed Practical Nurse program with a minimum of three years experience and evidence of progressive responsibility. Current, Unrestricted license to practice as a Licensed Practical Nurse in State of Florida.
LANGUAGE SKILLS: Ability to read, analyze and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, consumers, employees and general public.
