Provides advocacy, coordination and continuity of evidence-based care for individuals and families in need through a collaborative process of triage, assessment, planning, and communication. Participates as a member of a multidisciplinary treatment team. Reports to the unit/department supervisor. Bachelor's degree in a Human Services related discipline and at least one year experience in the mental health field (child welfare programs require two years experience with at risk populations and a child welfare certification). Ability to analyze, interpret clinical records, procedures or governmental regulations. Ability to write reports and document clinical activities and effectively communicate verbally with, provide information to, and respond to questions from managers, patients, service providers, mananged care entities, family members and other stakeholders. Must be capable to lifting 50 pounds of dead weight alone. May be required to complete standing presentations. High-paced, collaborative team performance with the emphasis on critical thinking and decision-making with guidance from program supervisor/manager. Some programs may require traveling within Hillsborough County via personal or agency vehicle to facilitate in-home client services in a variety of socioeconomic neighborhoods.
